Wellness Services Director/Area Mentor

Holbrook Life Management is seeking a dynamic Wellness Services Director/Area Mentor to join our team. This unique dual role combines community-based leadership with area-wide mentorship across our assisted living communities.

Qualifications:

• Licensed LPN or RN in Georgia
• Minimum 3 years' experience in senior living
• Strong leadership, mentoring, and clinical skills
• Proficiency in senior care management software (PointClickCare and QuickMAR preferred)
• Willingness to travel (25% of time) to support multiple locations

• Primary Location: Holbrook of Sugar Hill, GA

• Dogwood Forest of Grayson, GA
• Holbrook of Decatur, GA
• Holbrook of Woodstock, GA

The successful candidate will…

Community-Based Wellness Services Director Responsibilities:

• Oversee daily operations of the wellness department at Holbrook of Sugar Hill
• Ensure compliance with state and federal regulations
• Manage wellness staff, including hiring, training, and evaluations
• Develop and implement health interventions for residents
• Follow up on resident change in condition in timely manner
• Document all interactions, teaching, review, needs, and occurrences in progress notes
• Conduct assessments and determine resident care appropriateness - for move-in, hospital returns, changes, and per DPH regulations
• Coordinate with healthcare providers and families
• Manage departmental budget
• Must be out of the box thinker and Nurse from the heart

Area Mentor Responsibilities:

• Provide guidance to Wellness, Memory Care, and Assisted Living Directors at Grayson, Decatur, and Woodstock locations
• Conduct regular site visits to identify areas for improvement
• Implement best practices across supported communities
• Facilitate cross-community learning and collaboration
• Assist in onboarding and training new directors
• Support implementation of company-wide initiatives
• Provide bench support for open positions or leaves of absence
