Area Director of Wellbeing and Life Services Programs

To provide ongoing observation, evaluation, intervention, education, and review of all Wellbeing and Life Services community programs and to ensure programs meet or exceed quality hospitality standards.

  • Assess & evaluate Wellbeing and Life Services in assigned Communities to identify areas for improvement.
  • Identifies clinical development opportunities & informs Holbrook Life Management & local Community leadership about clinical “best practices.”
  • Completes and submits visit reports & audit results to Holbrook Life Management and Community’s General Manager.
  • Acts as a mentor & coach for Wellbeing and Life Services, Enriched and Inspired Living Directors for success at local Communities.
  • May need to assist the General Manager in reviewing candidates for Wellbeing and Life Services management positions.
  • Schedules Community visits based on Community issues and compliance needs.
  • Schedules training / professional development meetings for Wellbeing and Life Services, Enriched and Inspired Living Directors no less than semi-annually.
  • Establishes quarterly/annual performance improvement goals together with local & regional Wellbeing and Life staff.
  • Promotes Holbrook Life Management’s core values/ethics, policies/procedures & hospitality standards of practice.
  • Ensures compliance with Holbrook Life Management policies/procedures, State/ Federal regulations and current hospitality standards of practice.
  • Provides assistance & support throughout the survey process including preparation for survey, writing a plan of correction & developing an action plan, and ultimately monitoring for compliance.
  • Assist in the training of Wellness and Life Services, Enriched Living and Inspired Living Directors new to the role and orientation of Directors new to the Community.
  • Assists in the development and overview of Community policies and procedures as assigned or requested.
  • Review and follow-up on community incident reports that are health or injury related and develop/monitor plans of correction in accordance with Holbrook Life Management policies and procedures; and/or local and federal regulations.
  • Audit Point Click Care, Relias, Quick Mar, OnShift records, progress notes and dashboards weekly or as needed or directed by Holbrook Life Management.
  • Reviews, updates and approves Community submitted DCH Self Reports prior to Community submittal to DCH.
  • Reviews cost factors and assists in offering consultation in cost controls through efficiency whenever possible.
  • Participates in the review of the At Risk and Quality Assurance Meeting minutes to ensure action plans are identified and are executed in assigned Communities.
  • Participate in the analysis of the resident census, and assessment and service plan forms from assigned Communities to measure quality indicators and confer with appropriate interdisciplinary staff on identified patterns.
  • Supports the successful implementation and roll-out of company-wide or community specific initiatives at the community level.
  • Participates in the internal audit process, monitor outcomes and action plan development.
  • Participate in the external audit process as assigned.
  • Available in the Community as requested by the Regional, General Manager or Wellness and Life Services Directors for consultation or assistance.
  • Participates in Regional Meetings weekly. May be in conjunction with Director of Operations.
  • May be required to act as interim Wellness and Life Services Director when needed.
  • Regional overnight travel at Company’s expense may be necessary, as company needs determined.

The successful candidate will…

  • Graduate of an accredited School of Nursing with an unencumbered license in State of Georgia
  • Maintain current state license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) per state regulations in the states that Holbrook Life cover and support
  • 3 years as a Director of Nursing, Administrator, or significant other management role in Long-Term care, preferably in assisted living which included hiring staff, coaching, performance management daily operations supervision, and counseling
  • 2 years providing multi-facility nursing and resident care support
  • Demonstration of competence in assessment skills, critical thinking, injections/medication administration, follow up and triage
  • Working knowledge of Georgia Assisted Living Community Regulations
  • Valid driver’s license
